2012 Winnebago Community Credit Union
Jean Kolodzik Memorial Scholarship Program


Winnebago Community Credit Union will award one scholarship to a high school graduate who
demonstrates community service and academic achievement. The award is for one (1) renewable
scholarship worth $500 for each year of attendance at a secondary education institution for up to
four (4) years.

Eligible applicants must:
    1) Be a primary, active member of WCCU or the child of an active member
    2) Be a graduating, high school senior
    3) Demonstrate a commitment to community service
    4) Attend an accredited secondary education institution and achieve a minimum 2.0 GPA
Scholarship applications will be available starting November 14, 2011 for the 2011-12 school year.
They will be available through high school guidance counselors and on our website
www.wincu.org. To qualify, applications must be postmarked or dropped off at any of our branches
by April 10, 2012. Winners will be announced on April 30, 2012.

All applicants must submit*:
    1) A completed, typed application
    2) Two letters of recommendation: one from a guidance counselor and one from a teacher
    3) A 100-300 word original, typewritten, double spaced essay
    4) Completed, typed Community Involvement Form
*Failure to comply with the requirements of submission may result in disqualification.

    This year’s question:
      “How will continuing your education affect your future?”
Handwritten or incomplete applications will be disqualified.

Each winner must agree to allow WCCU to use his/her name and/or photograph in our promotional
materials. Decisions of the judging panel are final. WCCU is not responsible for lost or
misdirected mail. All submitted materials become the property of WCCU.

*Based on demand, the Credit Union would reserve the right to withhold the award of any of the scholarships.

WCCU employees, directors, committee members and their immediate families are not eligible.



SCHOLARSHIP APPLICATION



Winnebago Community Credit Union is proud to offer one renewable scholarship beginning with the 2011 - 2012 Academic Year for $500 each year of attendance at a secondary education institution for up to four (4) years. The scholarship is made possible by the Jean B. Kolodzik Memorial Scholarship Fund. Any senior who is a member or who has a parent/legal guardian that is a member of Winnebago Community Credit Union is eligible to receive the scholarship. Applications must be submitted through your high school guidance office and received by the Credit Union no later than April 10, 2012. The Board of Directors will decide on the recipient of the scholarship.
